I have messed up mechanics several times but I never submitted a ticket for it. Mostly I break low level dungeons when I run a guild mate through it but I have broken a raid boss or two.
My most recent dungeon misadventure was in one of the scarlet crusade dungeons where you need to carry the archery targets. I set my pet to attack the boss at the end and he died before our targets were knocked out of our hands so we were stuck carrying them and couldn't drop them.
We had to leave the dungeon and come back. most of my low level guild mates know to expect something to break if I run them through a dungeon.
